At approximately 11:38 pm ET on June 6th, an outage in our Azure infrastructure resulted in the inability of our users to load or submit forms. The outage was resolved at approximately 6:55 am on June 7th. All form submissions during this time were unfortunately lost, however, the forms themselves remain intact.
